- Those who wish to obtain application forms by post should send a self – addressed, stamped(to the value of Rs.10.00) envelope of size 9″4″ to the under mentioned address before 19th March 2018.
- Personal callers may obtain the application forms at the inquiries counter of the Department of Examinations, Pelawatta, Battaramulla up to 05th April 2018.
